REACH Kidney Care is a division of Dialysis Clinic, Inc. (DCI) that is responsible for leading healthcare innovation, including testing and implementing new ways to improve care for people with kidney disease. The REACH Patient Engagement Specialist is a critical part of the REACH Care team. You will work with REACH clinicians (nurses, dietitians, pharmacists, and social workers) who are located in field offices across multiple states and cities. REACH values the development of a collaborative relationship with these team members in a virtual environment. You will interact with eligible patients over the phone to help them understand how REACH can work with them to improve their kidney health. You will also support the REACH Care team by gathering, recording, and sharing important patient information to enhance the quality of care we provide to patients.
Schedule: Fulltime 40 hours/week; shifts typically include eight-hour days Monday through Friday.
