REACH Kidney Care is a division of Dialysis Clinic, Inc. (DCI) that is responsible for leading healthcare innovation, including testing and implementing new ways to improve care for people with kidney disease. The Engagement Team (ET) Specialist plays a critical role in connecting eligible patients with REACH kidney health management services through telephonic patient outreach and medical record data entry. Patient outreach focuses on educating eligible patients about REACH, answering their questions, and connecting them with the REACH clinical care team, including scheduling appointments. The primary purpose of the data entry function is to obtain key clinical information from other providers that will support better and more personalized care for eligible patients. Success in this role directly contributes to more eligible patients having the opportunity to receive individualized kidney health management from the clinical care team.
Schedule: Full-time 40 hours/week; shifts typically include eight-hour days Monday through Friday. Remote work with virtual meetings.
