REACH Kidney Care, a division of Dialysis Clinic, Inc., is a kidney health management program designed to benefit patients along the continuum of kidney disease through health education, self-management, behavior change counseling, coordination of care with other providers, and patient navigation services. This program includes assisting patients with advanced chronic kidney disease (CKD) and end-stage renal disease (ESRD). The goal of the REACH care team is to educate, support, and empower self-care to improve kidney health and delay the need for renal replacement therapies (RRT). For people who do progress to ESRD, the REACH care team aims to increase preemptive renal transplants (before the need for RRT), facilitate a successful transition to RRT or medical management (MM), increase renal transplant rates after the start of dialysis, and address advance care planning.
The REACH Care Manager provides remote (telephonic and video conference) education, care coordination, and navigation services for people with advanced kidney disease. This role empowers patients in self-management of their condition and following their current kidney care provider’s treatment plan. REACH goals for better kidney health include supporting an informed patient-centered treatment choice and subsequently at optimal transition. The REACH Care Manager coordinates care and communicates effectively with partnering nephrology providers as well as other members of the REACH multidisciplinary care team. Success is defined by patient engagement, patient-centered treatment choices, and optimal transitions to kidney failure. This position will reside and cover our locations in Montana.
Schedule: Full-time
Compensation: Pay range from $40 per hour, depending on nursing and dialysis experience
