The Renal Social Worker, in consultation with the attending physician, is responsible for assessing the psycho social and financial needs of peritoneal, hemodialysis, home, and kidney transplant and pre-ESRD patients; counseling patients and their families on ESRD related issues including financial and transportation needs.


This position is part time, 24 hours per week


The following is a summary of what makes our team successful. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 

  • Initially assesses the patient's psycho-social needs by collecting subjective and objective data and utilizing that information to develop and implement a plan for the provision of optimal care.
  • Counsels patient and families regarding ESRD therapies and the financial aspects of ESRD modalities,
  • Assists in the completion of financial forms and supplies transportation information for the patient.
  • Gathers and documents in the patient's medical record data to assess and modify the care plan in accordance with regulatory requirements on an on-going basis.
  • Compiles and develops educational materials to be used as teaching aids in education of patients and staff.
  • Participates in the care planning process for patients.
  • Maintains a professional working relationship with other staff of DCI.
  • Participates in other staff programs as required. 



Education and/or Experience:

  • Must be a Licensed Clinical Social Worker.
  • A Masters of Social Work Degree is a plus, but not required.
  • Dialysis experience a plus.
