The PD Home Training Coordinator is responsible for coordinating ongoing care of home patients and services provided. Coordinates training and modality selection of new ESRSD patients.


The following is a summary of what makes our team successful. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.


  • Assess new patient home situations to ensure they are appropriate for home therapies. 
  • Works as a liaison to discuss home dialysis as modality prior to initiation of dialysis therapy.
  • Oversees training of home patients in home hemodialysis, CAPD, or CCPD.
  • Works with Nurse Manager at hemodialysis clinics to coordinate any changes in patient’s modality (i.e. temporary return to hemodialysis).
  • Follows patients’ progress including any hospitalizations and hospital discharges to ensure continuity of program.
  • Schedules home visits as indicated, documenting the visit and findings.
  • Rounds with physicians and ensures orders are entered in the MIS and implemented.
  • Coordinates admission of new patients:  securing consents, reviewing pertinent policies and procedures, providing initial education.  Assigns new patient to primary nurse.
  • Identifies and coordinates patient procedures such as access interventions and physician office visits and makes appropriate referrals to dietary and social services.
  • Documents appropriately, according to clinic and DCI guidelines.
  • Provides direct patient care as needed.
  • Interacts with hospitals and physicians to assure continuity of care.
  • Ensures care provided to patients follows clinic procedures.  Takes corrective steps when procedures are not followed.
  • Participates in the clinic CQI program as assigned by the Nurse Manager.

Supervisor Responsibilities may include: assisting Nurse Manager in interviewing and hiring new employees; planning, assigning, and directing work; performance evaluations; addressing complaints and resolving issues; scheduling employees to meet patient census needs; training employees in Hemodialysis and Peritoneal Dialysis



  • Completion from an accredited Registered Nursing Program
  • Must possess and maintain current license in the state as a Registered Nurse
  • Maintain certification in CPR
  • Twelve months experience in a dialysis is preferred
  • Experience in critical care nursing is preferred
  • Experience in a supervisory role is preferred
  • DCI provides comprehensive hands-on and theory orientation/training in order to equip our nurses for success.
