As a Home Program Coordinator (RN) you educate your patients on how to do their own dialysis from their homes, and you ensure their ongoing treatment stays on track. Through this experience, the trusting relationships you build with patients and their families are strong and fulfilling.  You will empower dialysis patients to live healthier lives by sharing your knowledge!

Responsibilities

The following is a summary of what makes our team successful. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

 

  • Collaborate with Physicians and other health services personnel in setting up individualized plans of care for each patient.
  • Train patients and family who have chosen peritoneal or home hemodialysis as their treatment modality.
  • Monitor patients and equipment during dialysis 
  • Teach patients to monitor own dialysis equipment.
  • Teach patients proper care between dialysis treatments to avoid infections, and instruct patients on matters of diet.
  • Provide counseling and emotional support to minimize patient anxiety.
  • Keep comprehensive records of the schedule and type of treatments as well as changes in the condition of the patient.
  • Maintain regular contact with home patients by phone or during follow up clinic visits.
  • Maintain a schedule that ensures each patient receives a clinic visit monthly.
  • Obtain monthly laboratory work to include routine adequacy testing.
  • Establish and maintain an ordering system that ensures the patient receives all supplies in a timely manner.
  • Participate in monthly Quality Assurance meetings.  
  • Work closely with other members of the IDT i.e., Social Worker and Registered Dietitian.

Qualifications

  • Minimum 3 months' experience in Home Hemodialysis and Peritoneal Dialysis required
  • Degree from accredited school of nursing required
  • Current RN license in state of practice
  • Experience in case management, home health, disease management, or hospice preferred
