As a PD Home Hemo Registered Nurse (RN) you educate your patients on how to do their own dialysis from their homes, and you ensure their ongoing treatment stays on track. Through this experience, the trusting relationships you build with patients and their families are strong and fulfilling.  Empower patients to live healthier lives sharing your knowledge!

Responsibilities

The following is a summary of what makes our team successful. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Collaborates with Physician and other health services personnel in setting up individualized plans of care for each patient.
  • Train patients (and/or family) who have chosen peritoneal dialysis as their treatment modality.
  • Monitors patient and equipment during dialysis and teaches patient to monitor own dialysis equipment.
  • Checks dialysis equipment and ensures it is operating properly, making adjustments or having them made by maintenance personnel.
  • Teaches patient proper care between dialysis to avoid infections, and instructs patient on matters of diet.
  • Provides counseling and emotional support to minimize patient's anxiety over condition.
  • Keeps comprehensive records of the schedule and type of treatment and changes in the condition of the patient.
  • Make contact with the peritoneal dialysis patient at least monthly by phone or during clinic visits.
  • Maintain a schedule that ensures each patient receives a clinic visit at least every 3 months.
  • Obtain routine laboratory work to include routine adequacy testing.
  • Establish and maintain an ordering system that ensures the patient receives all supplies in a timely manner.

Qualifications

  • A.D.N degree from accredited school of nursing required
  • Current RN license in state of practice
  • Current CPR certification (or certification obtained within 60 days of hire)
  • Minimum of 12 months nephrology nursing experience preferred
  • 3 months' experience in PD
  • Experience in case management, home health, disease management, or hospice preferred
