    For REACH CKD patients and DCI ESRD patients, the ESCO Care Coordinator (RN) provides in-hospital, in-center and/or home coordination of care for the chronically ill, high-risk older adults hospitalized for common medical and surgical conditions.  In addition, the CC focuses on helping the patient and family caregivers develop the knowledge, skills and resources essential to prevent future decline and re-hospitalization. This CC will pay special attention to the patients desiring transplant, and help to coordinate the system through education, transplant referrals, coordinating appointments and following up with the patient and the family as needed.

    Responsibilities

    The following is a summary of what makes our team successful. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

    • As the primary coordinator of care, the ESCO Care Coordinator will assist with continuity throughout the entire episode of care
    • Help patients manage health issues, prevent decline and identify new risk factors
    • Work with DCI Pharmacist on medication reconciliation and management
    • Communicate with primary physician/nephrologist that his/her patient has been admitted to the hospital or desires transplant
    • Visit assigned patient who is admitted to hospital within 1 business day to review chart
    • Prior to hospital discharge, confirm office visit with primary Nephrologist’s office has been scheduled
    • Visit patient in home or clinic post discharge from hospital, per pathway
    • In order to establish continuity of medical care between hospital and primary care physicians, the CC will accompany patients to first follow-up visit, if patient needs assistance
    • Comprehensive, holistic focus on each patient’s needs including the reason for the primary hospitalization as well as other complicating or coexisting events
    • Active engagement of patients, their family members and informal caregivers including education and support during and after hospital admission
    • Early identification and response to health care risks and symptoms to achieve long term positive outcomes and avoid adverse and problematic events that lead to re-admissions
    • Communication/Transmission of order changes and discharge summary from the hospital to the dialysis clinic
    • Daily monitoring of hospital admissions for patients at assigned clinic(s)
    • Collaborate with local transplant professionals to create best practices for care coordination to improve transplant coordination and outcomes

    Qualifications

    • Completion of and certification from an accredited Nursing Program
    • Minimum of 2 years CKD experience, Care Coordination and/or Case Management preferred
    • Must complete DCI’s comprehensive training program
    • Currently licensed in the state of operation                                    
    • Proficient in Microsoft Word, Excel and PowerPoint
